
The term ‘Human Factors’ relates to how a human interacts with processes, systems, equipment and the environment. Applying HF principles to incident investigations is key to developing a positive safety culture by examining systems and identifying elements that contribute to errors and not just focussing on the individual(s) involved
The BHNOG human factors toolkit was devolved in response to the recommendations outlined in the 2018 SHOT Annual Report. Following a subsequent review of the BHNOG toolkit it was recognised that the SHOT Human Factors Investigation Toolkit (HFIT) is more frequently accessed by BHNOG members, therefore it has been agreed to withdraw the BHNOG toolkit and instead signpost users to the SHOT human factors resources and HFIT (link below).
As part of this review, the BHNOG Wrong Blood in Tube (WBIT) investigation form—previously embedded within the BHNOG toolkit was deemed preferable to the SHOT version and has therefore been retained, reviewed, and updated. The revised WBIT investigation form (version 2.0) is available via the link below on the BHT SharePoint pages.
